We want to try to avoid \u201cmini-lectures\u201d in the DCs. Instead, we want to teach by asking questions and thereby get students actively engaged in the learning process. The 5 microskills of the OMP are a handy tool to do that.<\/p>\n\n\n\n<ul>\n<li><a href=\"https:\/\/wpcdn.web.wsu.edu\/wp-hub\/uploads\/sites\/2950\/2022\/07\/Neher-et-al_1992_A-Five-Step-Microskills-Model-Of-Clinical-Teaching.pdf\">5 Microskills<\/a>&nbsp;<em>(Required article)<\/em><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Clinical Reasoning<\/h4>\n\n\n\n<p>Judy Bowen\u2019s NEJM paper on teaching clinical reasoning does a very good job of explaining the importance of the CASE REPRESENTATION\/PRESENTATION \u2013 something we want to try to emphasize when we interact with our DC groups.&nbsp; We will be expecting our groups to present their case each time they meet with us.&nbsp; We will then work around the circle and ask clinic members to also summarize <strong>{a}<\/strong> their thinking on the case (DfDx, etc), <strong>{b}<\/strong> their developing diagnostic plan, and <strong>{c}<\/strong> their planned interaction with their client. If you are planning to receive RACE CE credit, an have not <strong>set up an account for RACEtrack<\/strong>, please do so, before the end of DCs (it makes this process easier).<\/p>\n\n\n\n<ul>\n<li><strong>Teaching &amp; Learning (5 Credits) &#8211;<\/strong>  This is what you will already be doing during the DCs \u2013 guiding students through the simulation. <\/li>\n\n\n\n<li><strong>Clinical (3 Credits) <\/strong>&#8211; As a facilitator you are required to do a majority of the following tasks: prepare your case, know your disease so you can facilitate student learning, and complete a \u201cNotes for Next Time\u201d document with your co-facilitator. This document describes what went well and what could be approved for your case so that in 3 years the next facilitator can implement those things into the case.  The ONLY addition to receive the credit is to write at least a 1-page reflection on your experience. Since this is your first DCs as a facilitator, we often like to pair you with an experienced person to help show you the ropes of being a DC Facilitator. The DC has typically about 5 different cases running simultaneously, where four &#8220;clinic teams&#8221; (students) are assigned to each case. Clients are paired with Facilitators, and each pair has two clinics. Everyone should have been prompted to either work online (web based) with Microsoft Teams or download the app onto your laptop\/computer\/ipad when I sent you an invite to the team (<strong>IF NOT<\/strong> please let me know). I do find having the app is easier to work with, which would require that you bring your device with you when you arrive in Pullman. Either way works (online or app) \u2013 just make sure you have logged into your team account soon as possible or if you are having trouble, please let me know ASAP so we can figure out a solution.<\/p>\n\n\n\n<p>We continue to use Microsoft Teams for a majority of the communication and digital files <em>(all facilitator material, client folders, and your clinic teams \u2013 will be using this)<\/em>.<\/p>\n\n\n\n<p>You will have <strong>2 Microsoft teams<\/strong> \u2013 one for the volunteers (Facilitators &amp; Clients) and the other for the students. I want to make sure everyone has access and feels comfortable navigating through the program.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Case Material Location<\/h4>\n\n\n\n<p>To find ALL the needed material and handbook, go to the <strong>DC Facilitators folder<\/strong> <em>(DC1 or DC2 depending on when you arrive) <\/em>with the padlock symbol.<\/p>\n\n\n\n<ol id=\"block-c9d5ab8d-2772-4f11-89b5-54303e17be19\">\n<li>Once you find the <strong>DC Facilitators folder<\/strong> &#8211; select the &#8220;File&#8221; at the top<\/li>\n\n\n\n<li>Look for the folder that is labeled <strong>&#8220;DC Case Material&#8221;<\/strong><\/li>\n\n\n\n<li>Then look for the folder that has your species and last name<\/li>\n\n\n\n<li>The folder that is labeled DC-Some number &#8211; has all your material for the case<\/li>\n<\/ol>\n\n\n\n<p>I made a training video (mp4) , it is in the DC Facilitator folders for help. Overall this has been very successful in achieving a level of confidence and understanding that our client volunteers were able to start with on Day 1 and continue throughout the program. These training modules have helped prepare them to be our DC Clients. Part of this training is knowing and staying in their character during the entire week. <\/p>\n\n\n\n<p>In your case material you will find your <strong>DC Client Day 1 Script <\/strong>(or it will be attached to an email sent with your client&#8217;s contact information), you are welcome to change the details, but please <strong>keep it in the same format,<\/strong> as we are using them during the Client training workshop Monday morning of DCs at 9:00 am.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote\">\n<p>Remember, that the clients <strong>DO NOT know<\/strong> what is wrong with their animal(s), and only get information on a day to day basis. They will include their SOAPs, but you can also request the SOAPs to be emailed to you, just communicate that in the beginning.&nbsp; This will also be the place they submit test requests &#8211; you can use this or not \u2013 I made this folder for the students to keep track of their Dx submissions.&nbsp; You can still ask for them to be emailed to you, but again, communicate your needs. For the purposes of DC, a high yield may also be a problem in which writing a SOAP is likely to lead to significant learning. \u201cHigh yield\u201d does NOT necessarily mean there are a limited number of potential causes.<\/p>\n\n\n\n<p><strong>Jeff Abbott\u2019s best practice:<\/strong>&nbsp; Go through the SOAPs in real time with the students during a face-to-face session (client not present).&nbsp; Students much appreciate this and it will likely save you time and effort.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote\">\n<p><strong>Ideally, no student should be SOAPING more than&nbsp; about <span style=\"text-decoration: underline\">2-3 problems<\/span> each night<\/strong>.&nbsp; Of course some problems are pretty simple and quick, and they\u2019ve SOAPed some problems before (like anemia, stress leukogram, increased liver enzyme) so these shouldn\u2019t take too long.<\/p>\n<\/blockquote>\n\n\n\n<p>It\u2019s often good, especially if the number of active problems is small, to have multiple (even all) students SOAP central problems in the case (e.g. nasal discharge in an animal with possible fungal rhinitis or cancer).&nbsp; After about 50 minutes, not much more is heard.&nbsp; More importantly perhaps, there is often another group or the other facilitators waiting.<\/li>\n\n\n\n<li><strong>Use the Debriefing Agenda:<\/strong>&nbsp; It\u2019s best for the facilitator and client to go through the agenda BEFORE the session so that everyone knows the itinerary\/plan.&nbsp; A pre-debrief discussion is also a great time to preview and discuss what feedback you want to give, and ask questions.<\/li>\n\n\n\n<li><strong>Use the appointment schedules to set both of your 2 groups<\/strong>. Start in the room with the earliest scheduled time for your clinic, then move to the next room for the next session. Please <strong>bring your client <\/strong>with you, as they may not be familiar with the college layout, and remember to sit in the <strong>back of the room<\/strong>.<\/p>\n\n\n\n<p><strong>Questions<\/strong> &#8211; Please allow the students clinics to ask their questions first.
